EquiTrust | E-Connect  Revised Financial Needs Analysis Form
Required January 15
A revised Financial Needs Analysis (FNA) form ET-2506(12-19) will go into effect January 15, 2020, and is available for use now on AppBuilder and E-Apps. The new FNA form applies to all states, with the exception of Florida where the form is unchanged.
The revisions are intended to enhance E-App interface, help customer comprehension, reduce errors and minimize processing delays.
The new 12-19 version must accompany all annuity apps received on or after 1/15/20.
State Variations • California & Massachusetts State-specific forms apply, and both are available on AppBuilder and E-Apps. California: ET-2506CA(12-19) Massachusetts: ET-2506MA(12-19) • Florida The currently required form is unchanged – Annuity Suitability Questionnaire- • New Jersey & Washington State-specific FNA forms no longer apply; use the generic version: ET-2506(12-19)
